Current Situation of the Epidemiology of HCC in Asia-Pacific

Because of the high prevalence of chronic hepatitis B (CHB) in the Asia-Pacific region and chronic hepatitis C in the western countries, primary liver cancer, in particular hepatocellular carcinoma (HCC) remains a significant global disease. It is among the top 3 causes of cancer death in the Asia-Pacific region,according to Professor Man-Fung Yuen from the University of Hong Kong ,who presented a report on the epidemiology of HCC in the Asia Pacific. He said that the incidence HCC had been static for recent decades in the Asia-Pacific region. With older age being a major risk factor, the incidence increases exponentially after age 40 years. Males have a higher incidence of HCC, with a male to female ratio of 3:1. He pointed out that CHB was the most common etiological agent accounting for 75%~80% of cases in most Asia-Pacific countries. On the contrary, chronic hepatitis C infection accounts for most of the cases in Japan, Singapore and Australia/New Zealand. What is regretful is that the survival rate has improved only slightly over the years in spite of the major advances in surgery, liver transplantation and newer medical therapies. It is related to the fact that the majority of patients present with advanced diseases, hence reducing the chance of curative treatment. With the expectation of declining CHB infection because of the implementation of universal hepatitis B virus vaccination in the late 1980s in most of the Asia Pacific countries, the incidence of HCC may fall in the coming two to three decades.  He also said that this might also be partly due to the decreased incidence of medical transmission of hepatitis C virus infection. However, some of the independent risk factors, e.g., transmission of HCV by injection drug use, obesity and diabetes may partly offset this decline.
